Safety Issues in Colombia
We'd suggest you check the UK FCO Advice on Colombia with regards to safety issues for travel in Colombia. As of January 2008, the FCO advises against travel to various places in this travel guide, including the Ciudad Perdida, San Agustin, Tierradentro, Capurgana and the San Bernardo Islands. The FCO advice also advises against travel to the Pacific Coast, though it does point out that the towns of Bahia Solano and Nuqui are deemed relatively safe, as long as you stay in the towns and do not venture far away. Tourists were kidnapped on hour from Nuqui in January 2008, and a group of tourists was kidnapped on the trek to Ciudad Perdida in 2003. Many people consider the FCO's travel advice to be unnecessarily cautious. We've made a rather crude map pointing out the regions in which the FCO advises against "all travel" and "all but essential travel" - marked in red on the map below are regions were FARC and other guerilla groups are at times present.

Map of Colombia advising where is deemed safe to travel and were are FARC (& other guerilla) strongholds - it is by no means fully accurate, and things can change rapidly.
Finally, here is a map of the malaria regions of Colombia:
